- verifiedNo more than 1 CMV on-road preventable accident in last 2 years
- verifiedNo major CMV preventable accidents in last 5 years
- verifiedNo more than 2 moving violations in last 2 years
- verifiedNo DUI/DWI within past 5 years (10 years if holding CDL)
- verifiedPass DOT physical and drug test
- verifiedNo drug related conviction within past 5 years (10 years if holding CDL)
- verifiedNo positive/refused drug tests in past 10 years with CDL-A/CLP
