This position serves as a vital support pillar within the health center, operating under the general direction of the Nurse Care Manager. The role is designed to ensure the seamless flow of patient care by managing referrals, scheduling, and data integrity while actively assisting the nursing staff. You will act as a communication bridge between physicians, team members, and patients, ensuring that clinical changes, diagnostic results, and symptomatology are accurately conveyed and documented.
Your day will involve a dynamic mix of clinical and administrative duties. You will manage patient referrals and pre-certifications, ensuring all clinical information is provided to specialists within strict timeframes. A significant portion of your time will be dedicated to maintaining accurate and confidential medical records, both in paper and electronic formats, including scanning lab reports and organizing data into automated systems.
Beyond documentation, you will interact directly with patients and callers, determining their needs and providing necessary information within the medical assistance scope. You may assist with ancillary testing, point-of-care testing, and the preparation of patient rooms and sterile instruments. If you are a designated certified medical assistant, you may administer subcutaneous and intramuscular injections under physician direction. Additionally, you will participate in population management initiatives, utilizing various templates and reports to improve patient outcomes and support quality improvement projects.
Candidates selected for an interview will be required to pass proficiency testing in Spanish within 30 days if bilingual. The role requires adherence to all organizational policies regarding ethical business practices and regulatory compliance. Successful candidates will be expected to attend mandatory job-related training, including vaccine boot camps, and maintain active certification in Basic Life Support.
