About the Role
As a Case Manager RN, you will be the vital link supporting patients and families through every step of their care journey. In this role, you will coordinate services, connect resources, and develop care plans that reflect each patient's unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.
Your day will involve guiding patients and families through program orientations, explaining rehabilitation philosophies and insurance benefits, and developing individualized treatment plans that honor personal recovery goals. You will collaborate closely with the Rehab Program Director and interdisciplinary teams to ensure seamless, patient-centered care, promoting HCA Healthcare's values of compassion, respect, and excellence in every interaction.
