About the Role
As a Foster Care Case Manager, you will play a crucial role in providing essential case management services to children within the foster and adoptive care system. This position exists to establish permanency for these children, ensuring their safety and well-being while strengthening family dynamics. You will be part of a transformative mission, working closely with children, families, and out-of-home care providers to tailor support that addresses unique needs, stressors, and coping mechanisms. Key Responsibilities
- check_circleProvide comprehensive case management services to children and families in the foster and adoptive care system
- check_circleDevelop and formulate permanency goals and concurrent plans tailored to the unique needs of assigned children
- check_circleConduct assessments of child abuse/neglect risks and ongoing safety of children
- check_circleAdvocate for assigned children in court proceedings and provide crucial testimony on their behalf
- check_circleSecure appropriate and least restrictive out-of-home placements for assigned children
- check_circleSchedule, coordinate, and facilitate Family Support Team meetings within prescribed time frames
- check_circleArrange for medical, dental, and routine health services for children in out-of-home placements
- check_circleConduct face-to-face meetings with assigned children and out-of-home care providers according to prescribed time frames
Requirements
- verifiedBachelor's degree in social work, psychology, counseling, or related human service field
- verifiedMinimum one year of experience providing family and children services
- verifiedValid Driver's License
- verifiedReliable transportation
