We are seeking dedicated Community-Based Case Managers to join our programs in Delaware County and Montgomery County, Pennsylvania. These immediate openings are for professionals passionate about making a tangible difference in the lives of youth and families within their communities.
In this role, you will partner with youth, families, and key stakeholders—including Juvenile Probation Officers, Children & Youth Services, and schools—to coordinate individualized treatment plans. Your day-to-day will involve providing ongoing support to help clients meet court-ordered conditions, assisting with school re-enrollment, job readiness, and accessing recovery services. You will travel throughout the assigned county to meet with clients and partners, maintain accurate case documentation, and attend court hearings when required.
