About the Role
As a Case Manager, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise to shape a future where care and compassion converge for truly meaningful outcomes. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions to ensure quality, timeliness, and service appropriateness.

Key Responsibilities

Requirements
- verifiedActive license in RN, SW, OT, PT, ST, or Rehabilitation Counseling
- verifiedAssociate Degree in Nursing (RN) or Bachelor's degree for other disciplines
- verifiedCCM® or ACM™ certification required or obtained within two years
- verified2 years of rehabilitation experience preferred
Nice to Have
BSN degree (for Nursing discipline), graduate degree (for other eligible licensed or certified health care professionals), 2 years of rehabilitation experience.
