Key Responsibilities
- check_circleCoordinate patient care from admission to post-discharge and oversee interdisciplinary plan-of-care decisions
- check_circleFacilitate weekly team conferences and coordinate all treatment plan modifications
- check_circleReview and analyze case management reports including Key Care Indicators to plan appropriate actions
- check_circleUnderstand commercial contract levels, exclusions, payor requirements, and recertification needs
- check_circleMeet with patients and families within 24 hours of admission to establish communication plans
- check_circleAssist patients with timely procuring and planning of resources to avoid discharge delays
- check_circleEnsure accuracy of discharge and payor-related information in the patient record
- check_circleParticipate in utilization review process including data collection, trend review, and resolution actions
- check_circleAttend Acute Care Transfer meetings to identify trends and collaboratively reduce transfers
- check_circleWork with interdisciplinary team to guide treatment plans based on patient needs and preferences
- check_circleComplete case management addendums and all required documentation within specified timeframes
- check_circleMonitor patient experience regarding quality, timeliness, service appropriateness, and payor expectations
Requirements
- verifiedActive license in RN, SW, OT, PT, ST, or Rehabilitation Counseling
- verifiedAssociate Degree in Nursing (RN) or Bachelor's degree for other disciplines
- verifiedCCM® or ACM™ certification required or obtained within two years
- verified2 years of rehabilitation experience preferred
Nice to Have
BSN degree (for Nursing candidates), graduate degree (for other eligible licensed or certified health care professionals), 2 years of rehabilitation experience.
