At Recovery Services of Northwest Ohio, the Outpatient Case Manager is a vital member of a collaborative treatment team dedicated to supporting individuals managing mental health, substance use, and dual diagnosis disorders. The team works in close coordination with site supervisors, therapists, nurses, and psychiatrists to provide comprehensive care. While the role is primarily office-based, it also involves engaging with clients in their community settings to ensure holistic support.
This position focuses on building rapport and trusting relationships with clients to help them navigate their recovery journey. You will assist clients in understanding their conditions, problem-solving barriers to care, and connecting them with essential community resources such as medical appointments, social security, Medicaid, therapy, and self-help meetings. The role also involves facilitating mental health and substance use disorder groups, managing caseloads, maintaining clinical documentation, and ensuring compliance with accreditation bodies like CARF and OMHAS.
