As a Case Manager, you will leverage your clinical expertise to manage resources and coordinate patient care from admission to post-discharge. You will oversee interdisciplinary plan-of-care decisions, ensuring that care and compassion converge for truly meaningful outcomes.
In this role, you will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experiences regarding quality and service appropriateness, and collaborating to reduce unnecessary acute care transfers. You will also play a key role in the patient discharge experience, assisting with resource planning to avoid delays and ensuring accurate documentation for payors and post-discharge providers.
