In this role, you will work with an interdisciplinary team to guide treatment plans based on patient needs and preferences. You will establish tentative discharge plans, facilitate team conferences, and monitor the patient experience regarding quality, timeliness, and service appropriateness. BSN degree (for Nursing candidates), graduate degree (for other eligible licensed or certified health care professionals), 2 years of rehabilitation experience.
Skills: CCM, Acm, RN, SW, Ot, PT, ST, Rehabilitation Counseling, Utilization Review, Case Management.
Education: Minimum of an Associate Degree in Nursing for Nursing discipline; Minimum of a bachelor's degree for all other eligible licensed or certified health care professionals; Graduate degree preferred for all other eligible licensed or certified health care professionals.
