As a Case Manager at Encompass Health, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise while contributing to the well-being of individuals in your community. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions to ensure truly meaningful outcomes.
In this position, you will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experiences regarding quality and service appropriateness, and ensuring accurate documentation of discharge and payor-related information. You will also play a key role in reducing Acute Care Transfers (ACTs) and assisting patients with timely resource planning to avoid discharge delays.

Education: Minimum of an Associate Degree in Nursing required for Nursing discipline; Minimum of a bachelor's degree required for all other eligible licensed professionals; Graduate degree preferred for all other eligible licensed professionals.
