The Case Manager serves as a vital advocate for clients and their families who are homeless or at risk of homelessness. The primary goal of this position is to increase the quality of life for these individuals and maintain their highest possible ability to function within the community. Working with a strengths-based and client-centered approach, the Case Manager collaboratively identifies and addresses personal, social, and life challenges.
In this role, you will be the first point of contact for clients, conducting initial intake meetings to evaluate individual and family needs. You will build a comprehensive service plan that includes housing and other related needs, while coordinating and monitoring services to ensure progress. The position requires active networking within the community, including attending monthly support group sessions to assist clients in their transition from homelessness to stable housing. You will also represent the Emergency Solutions program by providing presentations at various community groups and events across both rural and metropolitan areas.
