As a Case Manager at Encompass Health, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions. Working closely with an interdisciplinary team, you will guide treatment plans based on patient needs and preferences, facilitate weekly team conferences, and monitor the patient experience regarding quality, timeliness, and service appropriateness. Your work ensures that patients and their families receive clear communication and that discharge planning is executed seamlessly to avoid delays.
