About the Role

This Case Manager opportunity brings your career closer to home and heart. As a vital link between exceptional care and the transformative impact on each patient's journey, you will leverage your clinical expertise to contribute to the well-being of individuals in your community. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions. Working with an interdisciplinary team, you will guide treatment plans based on patient needs and preferences, facilitate team conferences, and ensure the accuracy of discharge and payor-related information. Your day will involve monitoring patient experiences, analyzing case management reports, and collaborating to reduce Acute Care Transfers. By meeting with patients and families within 24 hours of admission and establishing clear communication plans, you will help ensure a seamless transition from acute care to rehabilitation and beyond.
