In this role, you will leverage your clinical expertise to manage resources and coordinate patient care from admission to post-discharge. You will oversee interdisciplinary plan-of-care decisions, guiding treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experience and quality indicators, and ensuring the accuracy of discharge and payor-related information. You will work closely with patients and families to establish communication plans, educate them on rehabilitation, and assist with timely resource planning to avoid discharge delays. Education: Minimum of an Associate Degree in Nursing for Nursing discipline; Minimum of a bachelor's degree for all other eligible licensed professionals; Graduate degree preferred for all other eligible licensed professionals.
