As a Case Manager, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise while contributing to the well-being of individuals in your community. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions.
In this position, you will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experience regarding quality and service appropriateness, and ensuring accurate documentation of discharge and payor-related information. You will also play a key role in the utilization review process and collaborate to reduce Acute Care Transfers (ACTs).
