About the Role

In this role, you will leverage your clinical expertise to manage resources and coordinate patient care from admission to post-discharge. You will oversee interdisciplinary plan-of-care decisions, guiding treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experiences regarding quality and timeliness, and ensuring accurate documentation of discharge and payor-related information. You will also work closely with patients and families to establish communication plans, educate them on rehabilitation, and assist with timely resource planning to avoid discharge delays.
