- check_circleEstablish tentative discharge plans and contingency plans
- check_circleFacilitate weekly team conferences and coordinate treatment modifications

- check_circleReview case management reports and plan appropriate actions
- check_circleMake appropriate referrals to post-discharge providers and physicians
- check_circleMeet with patients and families within 24 hours of admission
- check_circleParticipate in utilization review processes including data collection and trend review
- check_circleAssist patients with timely resource procurement to avoid discharge delays
- check_circlePerform goal assessments and complete case management addendums within 48 hours
- check_circleGuide treatment plans based on patient needs and preferences
- check_circleMonitor patient experience regarding quality, timeliness, and payor expectations
Requirements
- verifiedActive license in RN, SW, OT, PT, ST, or Rehabilitation Counseling
- verifiedAssociate Degree in Nursing (RN) or Bachelor's degree for other disciplines
- verifiedCCM or ACM certification required or within two years
- verified2 years rehabilitation experience preferred
Nice to Have
BSN degree (for Nursing candidates), graduate degree (for other eligible licensed or certified health care professionals), 2 years of rehabilitation experience.
