About the Role

As a Case Manager at Encompass Health, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise to shape a future where care and compassion converge for truly meaningful outcomes.

You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions. Working closely with an interdisciplinary team, you will guide treatment plans based on patient needs and preferences, facilitate weekly team conferences, and ensure the timely execution of discharge experiences. Your work will directly influence the quality, timeliness, and appropriateness of care, ensuring that patients and their families receive the support they need to thrive during their rehabilitation journey.
