About the Role

The Care Manager serves as a vital leadership figure dedicated to delivering high-quality, client-centered care. This position is designed to coordinate, implement, and supervise care services while fostering strong relationships with clients, families, and referral partners. The role acts as the primary bridge between the care team and the community, ensuring that every client receives personalized support.

Day in the Life

A typical day involves a dynamic mix of direct client interaction and operational oversight. You will conduct new client assessments and visit clients across various settings, including homes, hospitals, and rehabilitation facilities. A significant portion of your time is dedicated to developing and updating individualized Care Plans that reflect the unique needs of each client.

Beyond client care, you will supervise, coach, and support caregivers to ensure the highest standards of service delivery. This includes providing ongoing training on critical topics like infection control and emergency procedures, as well as conducting quality assurance visits. You will also serve as the main point of contact for families and external providers, coordinating care with home health agencies and hospice services.

The role requires flexibility and readiness. You will participate in an on-call rotation to support clients and caregivers during evenings and weekends, responding to urgent situations such as staffing gaps or care issues. Additionally, you may step in as a backup direct care provider or assist with office operations and scheduling as needed.
