As a Care Manager (MSW or LCSW), you will partner with patients, families, and the health care team to address and advocate for patients' social and emotional needs.
In this role, you will provide a full range of social work services, including psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. You will work alongside interdisciplinary teams to improve patient care through the effective utilization and monitoring of health care resources. Guided by the NASW Code of Ethics and the Swedish Mission and Vision, you will assume a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.
