We are seeking dedicated professionals to join our Inpatient Care Management team at Providence Regional Medical Center Everett. Whether as a Licensed Clinical Social Worker (LCSW) or a Master of Social Work (MSW) Care Manager, you will provide professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment.
Your mission is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral, or legal barriers to accessing necessary resources. You will work closely with the care team to assess needs, plan interventions, coordinate care, manage discharge planning, and advocate for patients. This role is designed for those who want to make a tangible difference in the lives of patients facing complex challenges, ensuring they receive the support they need to navigate their healthcare journey.
