This position is based in our North Central office in Bristol, serving the New Britain and Hartford communities. The Care Manager plays a pivotal role in coordinating the efforts of formal and informal caregivers to support individuals in maintaining an interactive process of informed decision-making regarding Long-Term Services and Supports.
