About the Role

This position is based in our North Central office in Bristol, serving the New Britain and Hartford communities. The Care Manager plays a pivotal role in coordinating the efforts of formal and informal caregivers to support individuals in maintaining an interactive process of informed decision-making regarding Long-Term Services and Supports.

Care Management at our organization is deeply person-centered, valuing the consumer's choices and rights above all. You will work to foster an environment where people of all ages, abilities, and incomes can live at home by identifying choices and providing necessary services. This role requires you to act as a bridge between clients, their families, and the broader community resources to ensure high-quality, cost-effective care delivery.
