About the Role
At the Sunrise of San Mateo community, the Care Manager (or Designated Care Manager) plays a vital role in providing the highest degree of quality care and services to a consistent group of residents and their families. This position is designed for those who find deep fulfillment in serving others and wish to use their unique skills to shed light on meaningful ways to serve, grow, and shine together.
In this role, you will provide hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP). Your day will be filled with the joy of creating pleasant days for residents, engaging them in life skills and enrichment activities, and maintaining a safe, comfortable atmosphere. You will be the consistent face of care, knowing where every resident is, observing changes in their condition, and responding promptly to their needs. Whether assisting with daily living activities, managing housekeeping and dining services, or simply offering a warm greeting, you will ensure that every interaction reflects our Principles of Service and Core Values.
