About the Role

At Sunrise at Orchard, the Care Manager (or "Designated Care Manager") is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living, long-term care, and reminiscence neighborhoods. This role provides hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment.

You will be a vital part of the team supporting our Mission, Principles of Service, and Core Values. Your day will involve demonstrating empathy and understanding each resident's unique communication patterns and history. You will strive to maintain an atmosphere of warmth, personal interest, and positive emphasis, ensuring a calm environment where residents can maintain their independence and dignity.

In the Reminiscence Neighborhood specifically, you will integrate the concept of 'Creating Pleasant Days' into the resident's daily routine, blending multi-sensory experiences and life skills into their day. You will engage residents in life enrichment activities, host daily activities, and participate in monthly letters home to connect residents with their families.
