About the Role
We are hiring a Case Manager/Care Manager for our Family Care Program in Milwaukee, WI. This rewarding opportunity is perfect for someone who enjoys team collaboration, community nursing case management, and being part of a mission-driven organization. You will join a team that shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community.
As we continue to grow our team, we are easing caseloads to bring even more work-life balance to our valued employees. This is a Monday through Friday position (8:00 a.m. to 4:30 p.m.) that offers a great work-life balance and an $8,000 sign-on bonus.

Requirements

- verifiedValid driver's license
- verifiedProof of required insurance coverage
Nice to Have
Current long-term care functional screener certification (eligibility for this certification is required). 3 years of work in community nursing.
