The SeniorCare PACE (Program of All-Inclusive Care for the Elderly) team provides comprehensive, patient-centered care to frail, elderly patients who qualify for nursing home-level care but prefer to remain in their communities. This interdisciplinary care model is designed to allow physicians to build meaningful relationships with patients, prevent hospitalizations, and improve the quality of life for vulnerable older adults.
We are seeking a Board Certified/Board Eligible Internal Medicine, Family Medicine, or Geriatrician to join our SeniorCare PACE team in Columbia, SC. As a primary care provider for PACE participants, you will conduct regular assessments and manage the complex medical needs of our patient population. Your day will involve working within an interdisciplinary team of nurses, social workers, therapists, dietitians, and other specialists to deliver coordinated care.
You will provide care across multiple settings, including the PACE center, participants' homes, hospitals, and skilled nursing facilities. A key part of your role involves performing detailed geriatric assessments that address cognitive impairment, functional decline, polypharmacy, and end-of-life planning. You will also participate in care coordination and family discussions to ensure care goals align with the participant's preferences and needs, collaborating with specialists and hospital teams when advanced interventions are required.
This position offers teaching opportunities with an academic appointment at the University of South Carolina School of Medicine, commensurate with experience.
