The Admissions Coordinator plays a critical role in facilitating timely and accurate hospice admissions by coordinating referrals, patient records, physician orders, and patient/family appointments. This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team.
In this role, you will be the compassionate and knowledgeable point of contact during the admission process, explaining hospice services, benefits, and eligibility to patients and families. You will ensure that the interdisciplinary team is promptly notified of new referrals and admissions, while coordinating the necessary supplies and DME to meet patient needs. Your work ensures that the transition into hospice care is seamless, dignified, and supportive for everyone involved.
