The Access Center Coordinator serves as the vital first point of contact for referral sources, patients, and families contacting HopeHealth. Under the direction of Access Center leadership, this role is dedicated to the timely and accurate processing of palliative and hospice referrals and admissions. The ideal candidate is passionate about end-of-life care, highly collaborative, and possesses a strong work ethic.
In this position, you will act as a liaison between patients, caregiving staff, and the billing department, ensuring seamless patient care through comprehensive chart management. You will provide first-line customer service by responding to calls, routing inquiries, and communicating patient status to community physicians. Your work ensures that referrals are complete, payer information is accurate, and DME supplies are coordinated effectively. You will also dispatch information and coordinate visit schedules for pre-admission staff, articulating all services offered by HopeHealth with clarity and compassion.
