About the Role
We are seeking a compassionate Hospice Chaplain for our Meridian, Idaho team. This role exists to provide spiritual care services to patients and families within the hospice program, either directly or through the coordination of care with other spiritual counselors. You will be an integral member of an interdisciplinary team, ensuring timely spiritual assessments and the implementation of individualized bereavement plans of care.
In this position, you will facilitate the development of care plans by participating in comprehensive assessments to meet identified spiritual needs. Your day will involve providing direct spiritual care, offering emotional, psychosocial, and spiritual support through bereavement counseling before and after a patient's death, and serving as a liaison to community chaplains and clergy. You will also recruit community spiritual volunteers and provide consultation and education to the interdisciplinary group on spiritual care matters.
