About the Role
We are seeking a Social Worker to join a dynamic, fast-paced start-up that is transforming the delivery of kidney care. This innovative care management position is dedicated to supporting patients suffering from chronic kidney disease and end-stage renal disease. You will drive to patients' homes, telehealth components, and work with ambiguity. The ideal candidate is mission-driven, willing to deal with underserved populations, and possesses the empathy to build strong relationships with patients and Community-Based Organizations (CBOs).
Day in the Life
This role involves significant travel, covering a two-hour radius to perform in-home care management visits. You will assess patients' social and behavioral status, identify social determinants of health, and develop plans to address them. Your responsibilities include delivering education on living with chronic illness, engaging family and social support groups, and facilitating conversations around proactive care decisions, including transplantation and home modalities. You will serve as the primary point of contact for patient questions regarding social and behavioral needs and act as a patient advocate, connecting individuals with appropriate resources. Additionally, you will document patient updates in the EMR and collaborate closely with the care team to ensure continual progress on care management goals.

Requirements
- verifiedMaster's Degree in Social Work, behavioral sciences, or related field
- verifiedLCSW or LMSW license
- verified2+ years experience in care management and/or chronic illness
- verified2+ years experience in medical settings such as home health, dialysis, or hospice
- verifiedAbility to work independently
- verifiedExcellent verbal communication skills
- verifiedFully vaccinated
- verifiedWilling to travel to patient's home
