We are seeking a dedicated team member to join our Continuum Complexivist department as part of our growing group of SNF Physicians. This role is designed for those who thrive in an integrated, compassionate environment where physicians, nurses, therapists, and social workers collaborate to provide high-quality, comprehensive care.
This position primarily supports the Portland, OR metro area, working across our core Skilled Nursing Facilities (SNF) and virtually through our Medicare Home Health Physician program. You will join an experienced, cohesive interdisciplinary team to manage transitions of care from the hospital to the skilled nursing setting, and from the SNF to home or other care settings.
Your day-to-day will involve caring for a spectrum of medical complexities, ranging from straightforward elective orthopedic surgery patients to those with multiple, complex medical problems. You will work alongside one to two other SNF physicians and a Kaiser Care Coordinator to achieve department goals, such as decreasing emergency room visits and re-hospitalization, and appropriately transitioning patients to palliative and hospice care.
The role offers a predominantly weekday, daytime schedule. While you will work most of the time at one of our 9 SNF locations, you will also support a virtual care model (roughly 10-20% of the work) that includes early, day, and evening shifts once per week. In this virtual capacity, you will support home health field staff in members' homes, handle physician consults for patient eligibility, and assist the "Transition Nurse Team" with escalations for patients recently discharged from hospitals or SNFs. Comfort with digital technologies, including AI platforms, telehealth, and Epic Health Connect, is essential, as is the flexibility to adapt to our evolving virtual care model.
