About the Role
As the Senior Care Healthcare Liaison, you will be the face of Oasis in the Twin Cities healthcare community. This role is built for a connector, collaborator, and catalyst who has spent time in the field, not just near it. Your mission is to build and deepen referral relationships across the care continuum, ensuring that when a patient or family needs placement support, Oasis is the first call their care team makes.
You will serve as a trusted resource for care teams navigating complex transitions, someone they call because they know you will get it right. From acute and post-acute care through long-term placement and end-of-life support, you will identify new outreach opportunities and represent Oasis at community events and professional networks. Every referral you make changes a family's trajectory, making this a role with a mission that truly matters.
Location Requirement
Candidates must live in the greater Minneapolis/St. Paul area to effectively manage the territory and build local relationships.
