About the Role

The Diabetes Nurse Educator utilizes a systematic approach to nursing, incorporating assessment, planning, implementation, and evaluation to provide high-quality care. This role serves individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing. You will act as a preceptor to support the development of other staff and healthcare students while providing leadership for initiatives designed to improve clinical outcomes.

In this position, you will facilitate competent healthcare delivery and ensure adherence to established standards of care. Your day-to-day involves completing clinical and educational assessments for all patients and developing individualized teaching plans focused on self-management, including diet, medication, and self-care. You will implement evidence-based clinical practice guidelines to provide diabetes education, including instruction on insulin pump utilization for all patients. Additionally, you will collaborate with all providers and members of the healthcare team to ensure comprehensive care and assist with community and organization projects.
