About the Role
This 100% remote position serves as a dedicated Health Coach and Diabetes Specialist, bridging the gap between clinical guidelines and daily member life. The role is designed to empower individuals to adopt healthier lifestyles through a collaborative, evidence-based approach. You will act as a primary liaison for members navigating chronic conditions, specifically focusing on diabetes management, while also supporting broader wellness initiatives.
In this capacity, you will move beyond simple information delivery to facilitate genuine behavior change. Your day involves conducting health risk appraisals, providing telephonic counseling on prevention guidelines, and developing targeted education programs for specific populations. Whether guiding a member through medication adherence or helping them set personalized nutrition and exercise goals, your work directly impacts clinical outcomes and self-management capabilities.
The position requires a blend of clinical expertise and community engagement. You will collaborate with internal stakeholders and external resources to ensure a seamless continuum of care, ensuring that every member receives the support needed to reduce complications and improve their quality of life. This role is critical in championing prevention, wellness, and disease management strategies within a diverse member population.

Requirements
- verifiedBachelor's degree in health education, Health Promotion, Nutrition, Nursing, Exercise Physiology or Social Work
- verifiedMaster's degree in related discipline preferred
- verifiedCertified Health Education Specialist (CHES) certification, Licensed Registered Nurse (RN), or American College of Sports Medicine (ACSM) certification
- verifiedCurrent unrestricted state of Michigan medical licensure
- verifiedValid and unrestricted Michigan driver's license
- verifiedTwo (2) years of experience in health promotion/education setting
- verifiedTwo (2) years of experience in delivering chronic condition coaching or diabetes education
- verifiedCertified Diabetes Educator Specialist (CDE) or Certified Diabetes Care and Education Specialist (CDCES)
Nice to Have
Master's degree in a related discipline. Certification or progress toward certification (such as CHES, RN, ACSM, CDE, or CDCES).
