The Registered Nurse Care Manager establishes a well-rounded care practice focused on patient assessment, care planning, implementation, coordination, and ongoing evaluation to ensure medically and financially appropriate care. This role is vital for facilitating interdisciplinary team care coordination and providing intensive medical management for participants recently discharged from hospitals and skilled nursing facilities.
In this position, you will work closely with the IDT Social Worker and Physician to collect necessary medical and social information. Your work ensures comprehensive decisions are made that prioritize the safety and well-being of participants.
