We are seeking a compassionate RN Care Manager to join our team in a PRN capacity at the Thief River Falls Clinic. This role is designed for nurses who want to utilize their expertise to coordinate care and improve health outcomes for patients in our community.
In this position, you will utilize the nursing process to provide individualized care, collecting and relaying critical assessment and diagnostic information to providers. You will develop and implement care plans, provide patient education, and coordinate services across the continuum of care by collaborating with inter-professional teams and community partners. The role involves applying population health management strategies to patient panels, managing disease registries, and ensuring adherence to evidence-based clinical guidelines.
