This position serves as a vital support partner for individuals with developmental disabilities, focusing on creating a nurturing and positive learning environment. The core mission is to help clients effectively cope with social, emotional, and physical daily situations while facilitating their independence. The role is active and dynamic, requiring engagement both inside the home and within the broader community.
You will be responsible for implementing Individual Plans of Care based on the interdisciplinary team's assessments. This involves documenting teaching strategies, tracking behavior plans, and reporting on client progress and barriers. A significant portion of the day-to-day life involves monitoring medical, physical, and behavioral changes, administering medications (once certified), and ensuring strict adherence to safety protocols and privacy policies.
Beyond direct care, the role encompasses household management, including meal preparation within budgetary constraints and maintaining a clean, safe living environment. You will also facilitate community integration by transporting clients to appointments and outings, fostering positive neighborhood relationships, and ensuring all activities align with the individual's support plan.
We are looking for individuals who possess a nurturing temperament, patience, and an optimistic energy. Success in this role requires the ability to communicate effectively with people at various ability levels and a genuine interest in improving the quality of life for others. Organizational skills are essential for managing programs, funds, and documentation.
A valid Connecticut driver's license with a good driving record and reliable transportation are required, as the role may involve utilizing a personal vehicle for client transport. While a high school diploma or GED is the baseline, experience in Human Services or a related Bachelor's degree is preferred. Candidates should demonstrate a willingness to learn about current issues in the field of developmental disabilities and maintain a self-motivated approach to their duties.
Interested candidates should submit their application to be considered for this per diem, as-needed position. The selection process will focus on matching your experience and temperament with the specific needs of our clients and the operational requirements of the role.
We are committed to fostering an inclusive environment where all individuals are treated with dignity and respect. We adhere to strict confidentiality policies and ensure that all team members understand and uphold the rights to privacy for every individual we serve.
