About the Role
We are seeking a dedicated Rehabilitation Specialist to assist individuals referred from CCBDD and Medicaid funding sources in meeting their basic living skills goals. In this role, you will serve as a vital liaison between participants, their families, guardians, and the broader support network. Your day will involve collaborating with co-workers to design and execute person-centered plans that foster daily living skills, personal growth, and social adjustment.
You will be responsible for developing vocational training curricula and adult day activities tailored to each individual's interests and needs. A significant portion of your time will be spent engaging participants in community activities and vocational outings, requiring you to drive an agency vehicle to facilitate these experiences. Beyond direct care, you will advocate for individual rights, maintain accurate electronic records compliant with Medicaid and CARF requirements, and ensure a safe, positive work atmosphere for all.
Hiring Process
Candidates must be willing to travel to other VGS locations and departments as needed to assist in providing services. The position requires a commitment to professional development, including a minimum of 15 hours of training annually through workshops, seminars, or RELIAS modules.

Key Responsibilities
- check_circleDevelop and implement programming to increase daily living and social skills
- check_circleCollaborate with co-workers to design and execute person-centered plans

- check_circleDocument community engagement and vocational activities in compliance with Medicaid and CARF
- check_circleWrite monthly progress reports and participate in annual or quarterly ISP meetings
- check_circleMonitor transportation for all enrolled individuals and assist with payroll documentation
- check_circleDrive agency vehicles to transport individuals on community outings and vocational activities
- check_circleMaintain electronic records including SMART goals, case notes, and incident reports
- check_circleManage a caseload of up to 12 individuals and track their progress and documentation
- check_circleEnsure participant rights are advocated for and maintain a safe work environment
- check_circleAssist individuals in developing job-keeping skills such as attendance and grooming
- check_circleCommunicate regularly with Support Administrators and families regarding individual progress
Requirements
