About the Role

This is an exciting opportunity to join a high-impact role where you can do work that truly matters. As a Registered Nurse Case Manager, you will be part of a team that values quality over quantity, maintaining manageable caseloads so you can focus on what matters most: care.

In this role, you will assume primary responsibility for your patient and family caseload. Your day will involve assessing physical, psychological, social, and spiritual factors to determine eligibility and initiate interventions. You will coordinate optimal care by communicating with attending physicians and the interdisciplinary team, perform regular visits to update care plans, and provide clinical pronouncements of death alongside family support. You will also supervise and document the delivery of care by Home Health Aides in compliance with Federal, State, and Company guidelines.

Hospice nurses are in growing demand nationwide, and RN case managers are among the most valued and respected roles in the field. We believe in promoting from within, with 80% of our nurse leaders starting in field roles.
