About the Role
We are seeking a compassionate and dedicated Registered Nurse Case Manager to join our newly restructured team in Las Cruces, NM. This is an exciting opportunity to be part of a high-impact role where you provide the highest quality care to patients and their families during life's most critical moments.\n In this position, you will manage a reasonable caseload, allowing you to focus on quality care rather than quantity. You will serve as the primary point of contact for your patient and family, coordinating with physicians and the interdisciplinary team to ensure optimal care. Your day will involve assessing patient needs across physical, psychological, social, and spiritual factors, updating care plans, and providing support during clinical pronouncements of death. We believe in promoting from within, with 80% of our nurse leaders having started in field roles.
