About the Role
In this field-based role, you will serve as a vital link for diverse populations, delivering skilled nursing care directly in patients' homes and community settings. You will perform comprehensive assessments, create and implement individualized care plans, and administer medications and treatments as prescribed. Beyond clinical duties, you will provide essential education to patients and families regarding disease management, medication adherence, and home safety.
You will collaborate closely with an interdisciplinary team to ensure safe, patient-centered care, documenting all interactions accurately in the electronic health record. Your work involves monitoring patient progress, recognizing changes in condition, and participating in quality improvement initiatives while adhering to strict safety and infection control standards.
