This role supports your unique calling in patient care within a clear pathway to advance. As a Case Manager, you will partner with physicians, nurses, and department leaders to support patients and families through every step of their care journey. Your mission is to coordinate services, connect resources, and develop care plans that reflect each patient's unique needs, ensuring safe transitions and consistent quality across the continuum of care.
You will guide patients and families through program orientation, explaining rehabilitation philosophies, insurance benefits, and discharge criteria.
