About the Role
This Ambulatory Care position is based at the Ascension Via Christi, St Francis facility in Wichita, KS. You will serve as a Licensed Registered Nurse, leading patient care delivery by implementing and monitoring personalized care plans that adhere to the highest clinical standards and safety protocols. As the primary coordinator across all clinical disciplines, you will advocate for patient outcomes, ensuring a seamless and holistic care experience.
Your day will involve driving clinical excellence by accurately executing physician and nursing orders while documenting the patient's journey to ensure high-quality record-keeping and continuity. You will also empower patient transitions by assessing and coordinating discharge needs with the multidisciplinary team, ensuring patients are safely prepared for the next stage of their recovery.
