Key Responsibilities
- check_circleProvide direct nursing care to patients in home and community settings
- check_circleConduct comprehensive patient assessments and monitor clinical status
- check_circleDevelop, implement, and update individualized care plans
- check_circleAdminister medications, treatments, and wound care per orders
- check_circleEducate patients and families on disease management and self-care

- check_circleDocument all care and assessments in the EMR accurately and timely
- check_circleParticipate in case conferences and quality improvement initiatives

Requirements
- verifiedCurrent RN license in state of practice

Nice to Have
ACLS certification. Home health or community nursing experience.
