This position serves as a vital bridge, linking individuals with Substance Use Disorder, opioid-related overdose survivors, and those facing health complications from substance use to essential care pathways. These include Medication Assisted Treatment (MAT), detoxification, and various inpatient and outpatient programs.\n You will spend your time communicating with patients in hospitals and the community to initiate treatment and engage them in ongoing recovery support. The role involves providing educational resources and community connections to participants, their friends, families, and caregivers, focusing on overdose prevention and recovery strategies. Additionally, you will conduct triage, screening, and evaluation services to determine the appropriate level of care for each individual, ensuring timely and professional documentation according to Operation PAR standards.
This role operates within a dynamic environment that includes treatment programs, mobile treatment facilities, and county jails. As part of a mobile MAT unit, you will be willing to work across multiple counties. The position may involve exposure to verbal abuse and patients with communicable diseases, requiring resilience and adaptability.
