This Case Manager position plays an important role in advancing our mission and supporting the communities we serve. You will conduct interviews with El Barrio students to assess barriers to employment and success with life goals, completing Individual Service Plans to guide their journey. Your work involves generating referrals to internal programs and external agencies for supportive services such as clothing, transportation, and childcare to remove or mitigate obstacles.
In this role, you will represent El Barrio at local meetings and conduct workshops on Case Management related topics, barrier removal, self-care, and supportive services. You will develop strong relationships with job seekers and support staff, assessing issues challenging success and making recommendations for improvement. The position requires maintaining accurate documentation, updating job seeker files, and ensuring data accuracy across all tracking and reporting systems.
This is an in-person role working Monday through Friday from 8:00 am to 4:30 pm or 8:30 am to 5:00 pm, with some evenings required. You will work with a diverse population, including young adults aged 18-24 seeking employment or education, and those aged 14-24 impacted by shelters, residential facilities, foster care, or the juvenile justice system.
