As a PRN Home Health Registered Nurse, you will provide coordinated skilled nursing care to patients of all age groups in their homes. In this position, you will assess home care patients to identify physical, psychosocial, and environmental needs. You will complete necessary documentation, including OASIS and visit paperwork, ensuring clinical notes accurately reflect communication with physicians and the care team. A key part of your day involves supervising and providing clinical direction to home health aides and LPNs/LVNs to ensure the continuity and quality of services. You will also participate in an on-call rotation to handle emergencies and significant changes in patient status.
