As a Primary Care Registered Nurse, you will be a vital member of our clinical team, facilitating the mission of Metro Community Health Center by providing direct clinical support to assigned providers. This role is designed for a nurse who thrives in a dynamic environment, functioning as an EMR champion and utilizing the nursing process to ensure quality, cost-effective care.
You will work both independently and collaboratively, managing a diverse patient population from ages 0 to 150. Your day will involve a blend of face-to-face and telephonic care, including telephone triage, medication management, and chronic disease education. You will play a key role in closing quality gaps, preparing patients for preventative care such as mammograms and colon cancer screenings, and assisting providers with complex procedures. Additionally, you will serve as a resource for patients, families, and staff, offering counseling and support while navigating barriers to health access.
The role requires a broad knowledge base in nursing theory and practice, with a specific focus on clinical applications, triage, and decision-making. You will be the first line of defense for immediate care needs, ensuring no sick child is turned away, and will actively participate in quality improvement initiatives to enhance patient outcomes and community health.
