You will deliver high-quality care in a collaborative, patient-centered setting, seeing patients across a variety of locations including home-based, facility-based, and outpatient clinics. Your day will involve providing direct patient care to a complex population, collaborating with multidisciplinary teams, and engaging in quality improvement initiatives. Additionally, you will have the opportunity to teach and mentor residents, medical students, and fellows.
